TELEHEALTH CONSENT & PRACTICE POLICIES

Telehealth Services

Telehealth allows you to receive medical care using video, phone, and secure messaging.

Telehealth offers convenience and access, but it has limitations. Physical examinations are not possible, and some conditions may require an in-person evaluation. If needed, you may be advised to seek in-person or emergency care.

Consent to Telehealth

By participating in telehealth services, you agree to receive care using electronic communication.

You understand that:

  • The same standards of care apply as in-person visits
  • Technical issues may affect the visit
  • Your information will be documented in your medical record

You may withdraw consent at any time.

Privacy and Security

Your health information is protected in accordance with applicable privacy laws, including HIPAA.

Telehealth platforms and communication tools used in this practice are configured to meet privacy and security standards. However, no electronic system is completely secure, and there is a small risk of unauthorized access.

This website uses privacy-first, aggregate analytics to understand traffic, referrers, devices, browsers, and basic site interactions. Analytics cookies are not used, and analytics events do not include personal health information or form responses.

Recording and AI-Assisted Documentation

Visits are not recorded except when an AI-assisted documentation tool is used to help draft clinical notes.

When AI-assisted documentation is used:

  • You will be informed at the start of the visit
  • Audio is processed only to generate a draft note and is not retained after the note is finalized
  • I review and edit every draft before it is added to your chart
  • Information is handled in accordance with HIPAA and the platform's business associate agreement

You may decline AI-assisted documentation at any time without affecting your care.

Communication and Patient Portal

The patient portal is intended for non-urgent communication only.

Messages are typically reviewed within 2–3 business days and are not monitored continuously.

The portal may be used for:

  • Administrative questions
  • Simple clarifications
  • Brief updates

The portal is not appropriate for urgent or emergency concerns. If you are experiencing a medical emergency, call 911. For urgent concerns, contact your primary care provider or seek urgent care.

Asynchronous Care

Some concerns may be addressed through asynchronous care, which allows your provider to review your message and respond without a scheduled visit.

This may be appropriate for:

  • Lab result review or ordering new lab tests
  • Minor medication adjustments

Asynchronous care is a clinical service and is billed $50 - $75 depending on complexity.

You will be informed of any cost before the service is provided.

More complex concerns, new symptoms, or situations requiring further evaluation may require a scheduled visit.

Scheduling and Visit Structure

Visit length is selected based on your needs.

Available visit types:

  • 45-minute visit
  • 20-minute visit

Longer visits are recommended for new patients and complex concerns.

If additional time is needed, your provider may extend the visit.

Payment and Fees

This is a cash-pay practice.

A deposit of $50 is required to book an appointment. Full payment is due at the time of service. Credit cards and HSA/FSA cards are accepted, if allowable per your policy.

This practice does not bill insurance. A superbill can be provided for possible reimbursement.

Charges may apply for:

  • Scheduled visits
  • Asynchronous care
  • Extended visit time

If you need to cancel or reschedule, please provide at least 24 hours’ notice. Appointments canceled or rescheduled with less than 24 hours’ notice will forfeit the $50 deposit charged at the time of booking the appointment.

Laboratory Testing

Laboratory testing is ordered as part of a clinical visit when appropriate.

If labs are obtained, follow-up is often recommended to review results and adjust your care plan.

You are responsible for completing the recommended labs and follow-up.

Prescriptions and Monitoring

Medications are prescribed based on clinical judgment, safety, best evidence, and patient needs.

Certain medications, including hormone therapy, may require:

  • Periodic follow-up visits
  • Laboratory monitoring

Refills may require a recent visit or updated labs.

Scope of Care

This practice provides specialty hormone and sexual health care and does not replace primary care.

You are encouraged to maintain a relationship with a primary care provider for:

  • Preventive care
  • General medical needs
  • Urgent concerns

If your needs fall outside the scope of this practice, you may be referred to another provider.

Emergencies

Telehealth is not appropriate for emergency or urgent medical concerns.

If you are experiencing severe symptoms—such as chest pain, difficulty breathing, severe pain, or other concerning symptoms—call 911 or go to the nearest emergency department.

For urgent but non-emergency concerns, please contact your primary care provider or seek urgent care.

Patient Responsibilities

By participating in care, you agree to:

  • Provide accurate and complete information
  • Participate in recommended follow-up and monitoring
  • Use the patient portal appropriately
  • Seek in-person care when advised
